Director of Pharmacy & Pharmacist - Pharmacy
Responsible for providing a productive and viable Pharmacy Department to support the program of clinical care. Assures the appropriate selection, dosing, administering, and dispensing of medication orders in the hospital. Serves as a subject matter expert in pharmacy discipline and provides guidance from pharmacy to all clinical departments.
Major responsibilities:
• Develops and conducts a productive and viable Pharmacy Department to support the program of clinical care by managing the daily operation of the Pharmacy and supervising staff as a working Director.
• Prepare and manage annual budget, staff interviews, hiring/separations, annual performance appraisal reviews of department personnel, staff scheduling, and time keeping.
• Provide counsel, where requested, to other leaders and staff on issues, questions and matters related to the Pharmacy.
• Provide clinical consultation and expertise in support of client assessment and care processes by consulting on drug therapy, drug regimen reviews and drug utilization reviews, consulting on drug storage and control.
• Participates in the development and conduct of clinical, staff, and continuing education programs by providing orientation for new staff members in the department and developing or assisting in the development of in-service education programs based on the needs of the staff.
• Manages the process of preparing, dispensing and monitoring needs of the staff medications by complying with prescriptions prescribed and ordered by a licensed physician and with applicable regulations and standards.
• Reviews direct oral or written doctor’s orders against patient medication profile for appropriateness of drug and dosage regimen.
• Maintenance of patient profile through computer order entry of new orders, order changes, etc. Maintenance of patient data (i.e. allergies) as required.
• Contacts and confers with physicians and nurses with regard to medication orders which are either unclear or otherwise irregular as it relates to the drug, dose, or drug relation to patient allergies or concurrent medications. Documents activity and follows up through resolution personally or confers with peers.
• Checks all new and routine medication orders compounded or filled by pharmacy technicians or students in a timely manner.
• Compounds and dispenses all dosage forms in a pharmaceutically elegant manner. Performs necessary calculations as required.
• Primary daily responsibility in assigned area is to prioritize tasks, control workflow, and make decisions as it relates to operations issues. Ensures appropriate order turnaround time and delivery of drug to patient. Addresses problems and works toward their resolution either personally or through conference with peers.
• Participates in all aspects of pharmacy CQI. This includes, but is not limited to medical error reporting, drug intervention documentation, data collection as required for Antibiotic Stewardship, and participation in decision-making regarding quality and operational issues.
• Assists in maintaining inventory control and proper duty of all pharmaceuticals (including controlled substances) as per department procedure and maintains a cost-conscious attitude in all aspects of the job.
• Monitor medication therapy for efficacy and safety in target patient populations as part of regular patient care unit-based practice.
●Administration of 340b program • Identify desired outcomes and contribute to the development of therapeutic plans.
• Intervene to prevent adverse drug events and medication errors.
• Ongoing collaboration with physicians, hospitalist, nurses, and other pharmacists is an essential facet of these processes.
• Performs other duties as assigned.
Qualifications: Graduate from an accredited School of Pharmacy with licensure as a Pharmacist in the State of Kentucky. Three (3) - five (5) years of experience in a hospital or retail pharmacy, one of which was in a supervisory capacity.
Reports To: Director of Clinical Services and Infection Prevention
Shift: Monday – 8:00 a.m. – 4:30 p.m. weekdays, weekend rotation and on-call requirement
